Implementation of the Family-centered Adolescent Sperm banking decision Tool (FAST)

Objective
Primary Objective:
Examine acceptability (reported by patients and caregivers) and effectiveness (by examining patient reported decision satisfaction) of the FAST as implemented at 3 sites to prepare for a larger multi-site hybrid implementation-effectiveness trial.

Treatment Clinicians at all sites will offer the FAST as part of clinical care during/after the fertility consult and before treatment initiation. Embedded at the end of the FAST will be a message that a research study is examining perspectives and experiences using the FAST as implemented, with the option to opt out of future contact regarding this research study.

Approximately one week after the FAST is fully or partially completed during/after a fertility consult, patients and caregivers who did not opt out of future contact during the FAST will receive an automated email and/or text message invitation with a link to complete the research survey in REDCap. One week after the initial invitation, patients and caregivers who did not opt out of future contact during the FAST and who did not fill out the survey will receive an automated email and/or text message reminder invitation.

Key Eligibility Clinicians at NCH, Riley Hospital for Children, and UW Health American Family Children’s Hospital will offer the FAST as part of clinical care, during/after a fertility consult and before treatment initiation. Participants who do not opt out of future contact for research (opt out mechanism embedded at the end of the FAST) will be sent a link to the information sheet and research survey approximately 1 week later. They will receive one reminder invitation if they do not complete the research survey after one week.
